It’s not nice to think about, but it’s important to make sure your family will be taken care of in the event of your death. The Company pays for:

• Employee Life Insurance up to two times your annual salary (up to $200,000)
• Employee Accidental Death & Dismemberment (AD&D) Insurance up to two times your salary (up to $200,000)

Basic Life and AD&D Insurance are available following the first of the month following your date of hire.

Company-paid disability coverage provides income replacement if you need to be out of work due to illness or injury. You are eligible for STD and LTD beginning the first of the month following your date of hire. 

• STD provides 60% per week gross income (up to $1,000) for the first 12 weeks following illness or injury. There is a seven-day waiting period before benefits begin. 
• LTD provides income replacement up to 60% of your annual salary up to age 65. It picks up where STD ends if you need to be out of work for an extended period.

Note that you do not need to enroll in Basic Life/AD&D, STD or LTD, as that coverage is provided automatically. You will need to make an active election when you enroll if you wish to purchase additional Voluntary Term Life or AD&D coverage for yourself, your spouse or your children. 

Important note about disability insurance for employees in California: Employees in California are entitled to California State Disability Insurance (CASDI) and are not enrolled in the Company-paid disability benefit.
